Motor vehicle crashes continued to account for the highest number of road traffic injuries and fatalities across the world in 2023, with a new global study revealing that nearly 51 million people suffered road-related injuries while around 1.34 million lost their lives during the year. Although the overall burden of road injuries has declined significantly over the past three decades, researchers cautioned that the benefits have not been shared equally, with low- and middle-income countries continuing to bear the overwhelming share of deaths.
The findings show that road injuries remained one of the world’s leading public health concerns. They ranked as the 11th leading cause of death globally and continued to be the principal cause of mortality among people aged between 10 and 39 years. For children in the age group of 5 to 9 years, road injuries emerged as the second leading cause of death, underlining the severe impact of traffic crashes on younger populations.
Researchers from the Institute for Health Metrics and Evaluation at the University of Washington analysed global road injury trends from 1990 to 2023. Their assessment found encouraging progress in reducing road injuries over the long term. Age-standardised rates of new road injury cases declined by 38.3 per cent during the period, while age-standardised mortality rates dropped by 32.3 per cent. However, the study noted that these improvements were concentrated largely in wealthier nations, whereas poorer countries experienced little measurable progress despite facing the highest burden.
According to the study, mortality rates from road injuries in low-income countries were almost six times higher than those recorded in high-income nations. More than 90 per cent of global road traffic deaths occurred in low- and middle-income countries, reflecting disparities in road infrastructure, vehicle safety standards, emergency medical services and enforcement of traffic regulations. Researchers said these differences highlight the urgent need for greater investment in road safety measures and trauma care systems in developing regions.
The analysis also examined the types of road users most affected. Motor vehicle crashes accounted for the largest proportion of both injuries and fatalities, resulting in nearly 19.8 million injuries and about 518,000 deaths worldwide in 2023. Pedestrians remained among the most vulnerable groups, contributing nearly one-third of all road injury deaths. Motorcyclists represented only 13 per cent of injury cases but accounted for almost one-fourth of all fatalities, indicating that crashes involving two-wheelers are often far more severe.
Researchers observed considerable variation in the nature and severity of injuries. Fractures emerged as the most common non-fatal injury, making up more than 38 per cent of all reported cases. Head injuries occurred in roughly one out of every ten road injury cases, with motorcyclists recording the highest proportion of traumatic brain injuries. Moderate to severe brain injuries were found to be especially common in low- and middle-income countries, where access to quality trauma care remains limited.
Beyond the human toll, the report highlighted the enormous economic burden imposed by road crashes. Researchers estimated that road injuries could cost the global economy nearly USD 1.8 trillion during the 2015-2030 period through healthcare expenditure, productivity losses and long-term disability. They suggested that strengthening trauma care systems, particularly in low- and middle-income countries, could save more than 200,000 lives every year.
The study comes at a time when global efforts to improve road safety are gaining renewed attention. The World Health Organization recently reported that despite a sharp rise in the number of vehicles worldwide, road traffic death rates have declined over the past decade. However, road crashes still claim more than one million lives annually and remain the leading cause of death among children and young adults aged 5 to 29 years. Health experts stressed that achieving the United Nations’ target of reducing road deaths and serious injuries by half before 2030 will require stronger road safety policies, safer transport infrastructure, improved vehicle standards and expanded access to emergency medical care across the world.
