Scotland, once regarded as one of the most violent countries in the developed world, has witnessed a remarkable decline in violent crime over the past two decades after adopting an unconventional strategy that treated violence as a public health issue rather than solely a criminal justice problem.
In the early 2000s, Scotland’s reputation was overshadowed by gang violence, knife attacks and soaring homicide rates, particularly in Glasgow, which recorded Europe’s highest murder rate between 2003 and 2005. At the time, people in Scotland were nearly three times more likely to be assaulted than those in the United States, prompting authorities to rethink traditional policing methods.
A defining moment came on October 24, 2008, when Glasgow Sheriff Court hosted an extraordinary gathering unlike a conventional court proceeding. Instead of criminal trials, around 85 rival gang members from Glasgow’s east end were invited to hear emotional testimonies from victims’ families, medical professionals and individuals personally affected by violence. The speakers described the devastating consequences of violent crime, urging those present to abandon gang life.
The initiative was organised by the Scottish Violence Reduction Unit (SVRU), established by Strathclyde Police in 2005 before being expanded across Scotland with government backing. Following the session, participants were provided with a confidential helpline to seek assistance in leaving violent lifestyles. Similar meetings were later conducted with hundreds of young people, with a significant majority reaching out for support.
The intervention formed part of a broader strategy that sought to address the root causes of violence rather than simply punishing offenders after crimes had occurred. Over the following decade, the results were striking. Glasgow’s homicide rate fell by 56%, while Scotland as a whole recorded a 38% decline in murders. Violent crime dropped by nearly one-third between 2006 and 2015, and the country now reports its lowest homicide figures in more than two decades. Serious assaults and attempted murders have also decreased substantially.
Today, Scotland records fewer murders per capita than several European nations, including Sweden, France, and England and Wales, marking a dramatic shift from its earlier reputation.
The transformation began when police analysts studying homicide data concluded that most killings were not linked to organised crime or carefully planned attacks. Instead, many resulted from spontaneous disputes, often fuelled by alcohol or personal conflicts, where a weapon such as a knife turned an argument into a fatal incident. This led officials to conclude that reducing homicide required addressing violence in all its forms rather than focusing exclusively on solving murders.
Former SVRU director Karyn McCluskey and analyst Will Linden played central roles in shaping the new approach. Supported by senior police leadership, the unit was given the freedom to experiment with innovative strategies, even at the risk of failure. The team began treating violence similarly to an infectious disease, emphasising prevention, early intervention and community partnerships instead of relying solely on arrests and prosecutions.
The public health model involved identifying people most vulnerable to violence through research and evidence. Authorities found that nearly two-thirds of violent incidents involved just one per cent of Scotland’s population. Young men from economically deprived communities, individuals facing unemployment, poverty and unstable family environments were identified as being at greater risk. At the same time, education, supportive families and positive social relationships emerged as important protective factors.
Using these findings, the SVRU developed targeted interventions ranging from mentoring programmes and educational initiatives to partnerships involving schools, healthcare professionals, social workers and community organisations. Each programme was evaluated before being expanded where successful.
Many of Scotland’s initiatives drew inspiration from successful violence prevention models overseas. The concept of addressing violence through a public health framework originated in the United States during the 1970s and later received backing from the World Health Organization, which recognised violence as a major global public health concern in 1996. Scotland adapted programmes from cities such as Chicago and Cincinnati while tailoring them to local circumstances rather than simply replicating foreign models.
Healthcare professionals became key partners in the campaign. Dentists received training to identify injuries linked to violence and direct patients towards support services. Schools also changed disciplinary approaches, with authorities encouraging efforts to keep vulnerable students in education instead of excluding them. As a result, school exclusions declined dramatically from nearly 45,000 in 2006-07 to fewer than 12,000 in 2022-23.
Medical professionals also expanded their role beyond treating victims. In 2008, oral surgeon Christine Goodall co-founded the charity Medics Against Violence after witnessing years of severe facial injuries caused by assaults. The organisation launched educational programmes in schools and introduced Hospital Navigators—trained support workers who engage with victims arriving at emergency departments following violent incidents. The programme aims to break the cycle of retaliation by connecting patients with counselling, housing, education and employment support.
Experts involved in Scotland’s transformation believe the country’s experience demonstrates that reducing violence requires coordinated action across policing, healthcare, education and social services. By treating violence as a preventable condition rooted in wider social circumstances, Scotland has managed to reverse decades of entrenched crime and establish itself as one of Europe’s safer nations, offering a model increasingly studied by governments around the world.
